Deep dive · Hospitals in Chennai

What makes hospitals and clinics payroll in Chennai genuinely different

A Chennai hospital payroll has 5+ employee classes that don't exist anywhere else: MBBS resident on stipend (under PG agreement), MD/MS consultant on per-OPD-visit + per-procedure incentive, full-time on-call surgeon on retainer + duty allowance, nursing on rotational 3-shift (8 AM / 4 PM / midnight), and support staff on monthly. NMC compliance requires retention of credential expiry dates (medical license renewal, registration with TN Medical Council). StafFixHR tracks all 5 classes from one tenant — rotational shifts, on-call duty calendar, per-visit payouts via OPD logger, license-expiry alerts, and the consolidated FNF includes pending consultant invoice reconciliation.

Built-in workflows for hospitals and clinics

The five things StafFixHR ships out-of-the-box that generic HR software makes you build yourself.

Rotational nursing shifts
3 shifts × 8 hrs × 365 days. We pre-config the cycle, auto-assign, and let staff swap with manager approval. OT computed per shift not per day.
On-call duty + duty allowance
Doctors on-call get ₹X duty allowance + ₹Y if actually called in. Roster shows who's on; calls are logged and trigger pay.
Per-visit consultant payout
Consultant logs OPD visits + procedures in the system; monthly settlement = visits × rate + procedures × rate. Paid as professional fee (Section 194J TDS, not 192).
License + credential tracking
MBBS / MD / nursing registration expiry tracked per employee. 60-day alert to HR before any license lapses.
NMC + NABH-aligned records
Employment letters in the format NABH inspectors expect; HR file is downloadable as one PDF per employee on demand.

Hospitals in Chennai — common questions

Answers HR ops at hospitals and clinics actually search for.

How does TDS work for consultants paid per-visit?
Consultants are professionals under Section 194J — TDS is 10% (or 2% if specified) on gross, not the salary slab rate (192). We compute 194J separately, file Form 26Q quarterly, and issue Form 16A (not 16) to the consultant. Many Chennai hospitals get this wrong; we don't.
Rotational nursing shifts with weekly off — how is that scheduled?
Configure shift cycles (morning / evening / night) + weekly-off pattern. Each nurse is auto-assigned by the cycle; she can request swaps with a colleague (manager approves). The roster page shows the next 30 days; nurses see their schedule on the mobile app.
TN Medical Council registration expiry — can the system alert us?
Yes. Each medical staff member has a 'registration expiry' field; we alert HR 60 days before, then 30 days, then 7 days, then on the day. Missed renewals create a payroll-hold flag so payroll doesn't roll forward without HR acknowledging the lapse.
Can we run nurses + doctors + admin from one tenant?
Yes. They're three different employee classes with three different salary structures and three different statutory profiles. Nurses → PF + ESI (regular employees). Doctors → 194J professional. Admin → regular. One Form 24Q for the regular employees, one Form 26Q for the consultants. All from one tenant.
How do you handle scheduled CME (continuing medical education) days?
CME days are leave under a custom 'Professional Development' leave type — counted as paid, doesn't deplete PL/CL, evidence (certificate) uploaded by employee. NABH inspectors look for this; we track it cleanly.
Doctor leaves to join competitor — non-compete clause enforcement?
We don't enforce non-compete (that's a legal/courts question), but the FNF flow blocks 'experience letter' issuance until HR signs off, and the audit log captures the entire FNF + non-compete acknowledgement trail.
